This Quick Reference Guide, will describe two ways to access Electronic forms using the ScreenSteps website.
The Electronic Forms can also be accessed from your mobile device.
Accessing Electronic Forms from MYDOHC.com site
Not all users who access the Electronic Forms use the NextGen program. In this case, users can do the following to access the Electronic Forms:
- Open Google Chrome Web Browser.
- Type in the following web address: www.mydohc.com/for-providers/
- Select the Enter (Return) button on your keyboard.
It is recommended that you save the link to your Browser Favorites list.
From the For Providers page: click the link next to Electronic Forms. (See Image Below)
You should now be presented with a list of Electronic Forms. (See Image Below)
Accessing Electronic Forms from NextGen EHR.
Electronic Forms can also be accessed from any Core template (i.e. Intake MG, SOAP MG, Telephone Call MG, etc.) within the NextGen EHR application. To do so:
- Select the Navigation Bar located on the Left hand side of the selected Core template.
- Click the DOHC Resources button.
The illustration below show the Navigation already expanded.
To access the Electronic Forms:
- Click the Electronic Forms and Surveys option.
Select Electronic Forms from the list below.
Select from the available list of Electronic Forms.
