Pre-Visit Planning

The Pre-Visit Planning template is used prior to patient visits and provides one place to view patient information. Care opportunities in the form of Work List can also be created so providers can focus on the patient.

Filters

The Information in the Pre-Visit Planning template can be filtered by the following:

  • Specialty - users have the option to view all specialties or a specific specialty.
  • Location - the following views are available: All, Current Location, and Encounter Location. Users can also choose a specific location.
  • Provider - the following viewing options are available: All, Current Provider, and Encounter Provider. Users can also choose a specific provider.
  • View - the following options are available: All, Last 3 Months, and Last Year.

Work List Panel

Care Guidelines

The Care Guidelines section is a view-only window that displays all guidelines that are associated with the patient. The following task can be performed from this section:

  1. Task - providers can assign tasks to be completed by clinical personnel.
  2. Care Guidelines - providers can access the Care Guidelines template to manage or add Care guidelines.
  3. Add to Work List - during the pre visit planning providers can select Guidelines that will be addressed during the patient's visit and Add it to the Care Guidelines Work List.

Adding Care Guidelines to Work List

To add Care Guidelines to the Work List:

  1. Select the guideline in the Care Guidelines display.
  2. Click Add to Work List

Note: The Care Guidelines Work List field is also free textable which allows the provider to document additional information. The My Phrases functionality is also available in this section.

Active Care Plans Work List

When the Care Management Plan/Intervention template is completed by care management personnel. Providers are able to see the active care plans in the Active Care Plans display window. The following options are available in this section:

  1. Refresh - refreshes the list of Active Care Plans to display any changes.
  2. Task - providers can assign tasks to be completed.
  3. Care Management Plan/Intervention - accesses the Care Management Plan/Intervention template which allows for editing current plans or adding new care plans.
  4. Add to Work List - during the Pre-Visit planning providers can select Active Care Plans that will be addressed during the patient's visit and Add it to the Active Care Plans Work List.

Note: The My Phrases functionality is also available in this section.

Other

Providers can also document additional needs to be addressed during the patient's upcoming appointment in the Other section. By clicking the check box next to each item to be addressed, the provider can add those items to the Work List. Providers can also write customized items in the empty text fields provided.

Assessment / Plans Panel

View Encounter

In the Assessment / Plans panel providers can view assessment and plan information by encounter. To select the encounter, click the drop down arrow next to the View Encounter field.

Select the desired encounter then click OK.

Providers may also use the Previous Visit and Next Visit buttons to view assessment and plan information by encounter. By clicking the double arrows the very first or very last encounter will be selected.

View Document

Providers can view various documents for the encounter (i.e. Office Visits, Chart Notes, etc.) by clicking on the View Documents button.

Note: This feature does not generate a document but instead allows you to view existing documents in the patient's chart.

Documents / Images Panel

Under the Documents / Images panel providers can view and bookmark all EHR and Document  Management documents for the selected practice.

Viewing EHR Documents

To view EHR documents, double click the document in the EHR Documents display window.

Viewing Document Management Documents

To view document management documents, select the document in the Document Management display window then click Preview.

Bookmarking EHR and DM Documents

Providers can flag or bookmark documents so that they will be readily available if needed during current and future Pre-Visit plannings. To  bookmark a document, perform the following steps:

  1. Select the document from the EHR Document / Document Management display window(s).
  2. Click the Bookmark button.

Note: To remove Bookmarks, select the document(s) in the Bookmarked display window then click Remove Bookmark.

Communications / Tasks Panel

The Communications / Tasks panel has four view-only displays:

  • Phone Calls - view telephone call history for current patient and selected practice.
  • Portal Communications - view Patient Portal communications for current patient.
  • Appointments - view all scheduled appointment information for current patient.
  • Outstanding Tasks - view incomplete tasks for current patient.

Note: Providers can assign tasks from this panel by clicking Task button.

Today's Orders Panel

The Today's Orders panel functions pretty much the same way as the Today's Orders panel located on the Checkout template. (See Checkout Template training materials for more details)

Lab Results

Lab results from the Orders Module are displayed in the Lab Results panel.

Note: Result comments can be viewed by hovering the mouse over the row of the results.

Problems List

Providers can view and manage the patient's problems list from the Problems List panel. The following options are available in this panel:

  • Resolve Condition - automatically resolves the selected problem with the current encounter date.
  • Refresh - updates the problems list display.
  • Add/Edit Problems - accesses the Problems Module where problems can be added or edited.

Note: Sorting and filtering options are also availabe at the top the Problems List display.

History

The History panel allows the provider to review, add or edit the following:

  • Medical / Surgical / Interim history
  • Family history
  • Social / Confidential history

Completing the Pre-Visit Planning

Once all pertinent information has been reviewed by the provider. Additional information can be added to the Work List if needed or the provider can simply just complete the Pre-Visit Planning. Perform the following steps to complete the Pre-Visit Planning:

 

  1. Return to the Work List Panel
  2. Click the Pre-Visit planning complete check box (located at the bottom of the Other section)
  3. Generate Document
