This Quick Reference Guide explains the proper steps for documenting the Influenza Vaccine.
Accessing Patients Chart from EHR Workflow Appointments Inbox
From the EHR Workflow Appointments Inbox, double-click the patient receiving the flu shot.
Perform 5 Point Check
Note: It is important that you perform the 5-Point-Check before documenting in any patient's chart. The 5-Point-Check helps to minimize errors that may occur while documenting.
The 5-Point Check is as follows:
- Confirm that you have the Correct Patient.
- Verify that you have the Correct Location (DOHC ORS PS, DOHC ORS IN, Etc.)
- Correct Provider (Hoffing, Marc MD)
- Correct Encounter.
- Correct Provider name on the Encounter.
Accessing the Intake MG Template
To access the Intake MG template from the Patient History panel,
- Select the Template Module icon.
- Select Intake MG from the menu.
- Click OK
After opening the Intake MG template, perform the following steps:
- Verify that Internal Medicine is selected as the Specialty.
- Select Nurse Visit as the Visit Type.
Reason for Visit
On the Reason for Visit panel,
- Click the Do not launch HPI check box.
- Select flu shot from the default list of reasons.
Documenting Patient Vital Signs
Proceed to the Vital Signs panel then click the Add button just below the Vital Signs grid.
Document the patient's temperature by performing the following steps:
- Free text the patient's temperature in the Temperature fields.
- Record the Site. (options available: axillary, ear, oral, rectal, and temp strip)
- Click Save
- Click Close
Celsius is automatically calculated based on the Fahrenheit value.
Accessing the Immunizations Template
The Immunizations Template can be accessed by selecting Immunizations from the left Navigation bar.
CAIR Consent
Click the CAIR Consent active text link to proceed with documenting the patient's consent to send a copy of the records to the California Immunization Registry (CAIR).
- Select Consent: Yes, No or Unknown.
- Click to Enter date consent recorded.
- Click Disclosure to print consent form. (English or Spanish)
- Click Save & Close.
Guilliain-Barre Syndrome
Ask the patient to be vaccinated if they have ever had Guillain-Barre syndrome. Based on patient's response, select Yes or No.
Create New Order
Click the New Order button on the Immunization Orders toolbar to begin ordering the Influenza vaccine.
Selecting the Influenza Vaccine
***NOTE: FOR DOHC EMPLOYEES AND EMPLOYEE FAMILY MEMBERS PLEASE SELECT DOHC Employee/Family Flu Vaccine TO AVOID A CHARGE.***
Select the indicated vaccine, by clicking on the check box located next to the appropriate Influenza description.
Review Allergies
Review patient's allergies in the Review Allergies pop-up. If the patient has no pertinent allergies, click the No To All Allergies check box, then Save.
Vaccine Details
To complete the order, click on the Vaccine Details tab located on the bottom left corner of the Immunization Order screen.
Print VIS
The Vaccination Information Sheet (VIS) can be printed in English and in Spanish.
Click Document As Given at the bottom of the Print VIS window to proceed to the Vaccine Details screen.
Completing the Vaccine Details
To complete the Vaccine Details for the Influenza Vaccine, click the drop down arrow next to Lot#.
***NOTE: PLEASE BE ADVISED THAT THE LOT# FOR DOHC EMPLOYEES AND FAMILY MEMBERS START WITH MG-***
Because we have implemented the vaccine inventory for the flu vaccine the following fields will auto-populate:
- Expiration date
- Manufacturer
- Brand name
- Vaccine date/time
- Amount
- Route
- NDC ID
- Billing units
- Billing Code
- Admin CPT4
Fields to be completed
The following fields will need to be completed by person completing the order:
- Site
- Consent was given from. (i.e. Self, Father, Mother, etc.)
- Any Comments/Notes. (i.e. Patient tolerated well, No adverse reaction, etc.)
- Click Print/Give VIS then click on Print.
- Click Save
Generating Master Document
After saving vaccine, select SOAP from the left Navigation bar.
Scroll down to the bottom of the SOAP template and perform the following steps:
- Click Generate Note
- Select Master Document from the menu.
- Click OK to generate document.
Once note has been generated go to FILE menu AND CLOSE PATIENT.
