The Screening Tools template is updated to allow you to document Elder Maltreatment using the Hwalek-Sengstock Elder Abuse Screening Test (H-A/EAST).
This measure calculates the percentage of patients aged 65 years and older with a documented elder maltreatment screening using an Elder Maltreatment Screening Tool on the date of encounter and a documented follow-up plan on the date of the positive screening.
To access the Hwalek-Sengstock Elder Abuse Screening pop-up template:
1. Click the Screening Tool active text link under Vital Signs located on the SOAP template.
The Screening Tool can also be accessed from the Health Promotion Plan template under the Functional Status panel.

Select Hwalek-Sengstock Elder Abuse Screening Test (H-S/EAST) from the Screening Tools template.

Complete all necessary fields as instructed then click Save & Close.
The Hwalek-Sengstock Elder Abuse Screening questions will be visible on the Master Document.
