How to Access the Wound Risk Calculator
Using the SOAP MG template:
- Click on Screening Tool link on the Vital Signs panel.
- Click the Wound Risk Calculator link.
Completing the Wound Risk Calculator
To complete the Wound Risk Calculator:
- Select one of the following options; Prevention Screening or Wound Risk.
- Complete the Screening questions.
- Click Calculate
- Click Submit
- Click Save & Close
After clicking the Calculate and Submit buttons, the Result will appear in the Score field.
Wound Risk Score
The Interpretation of the Score can be retrieved by accessing the Wound Risk Score template. To access this template:
- Click the Blue informational button next to the Wound Risk Calculator link located on the Screening Tool template.
The Wound Risk Score and interpretation will be added to the Screening Tool grid and will also display on the visits Master Document:
