How to Document the PPD Skin Test for Tuberculosis

This Quick Reference Guide assumes that the user has a basic understanding as to how to access the Intake MG template.

How to Access the Tuberculin Skin Test MG Template

Using the left Navigation MG panel from the Intake MG template; select Tuberculin Skin Test from the menu.

How to Place an Order for the PPD Skin Test

To place an order for the PPD Skin Test:

  1. Click the PPD check box
  2. Complete the following fields (Side, Lot#, Manufacturer, and Exp. date).
  3. Click the Performed now check box.
  4. Click the Place Order button.

With the exception of the Location and Schedule fields. All other fields will auto populate PPD information.

How to Submit the 86580 CPT Code

The 86580 CPT Code is used for skin testing to determine if a person has TB. It is not used for administering a vaccine. The 90471 for vaccine administration is not to be reported with the 86580 CPT Code.

To Submit the 86580 CPT Code:

  1. Double click the Order in the Tuberculin Skin Test grid.
  2. Un-check the Result read check box.
  3. Click the Submit to Superbill button.
  4. Click Save and Close

How to Generate the Intake Note

The Intake Note must be generated after submitting the 86580 CPT code to the Superbill. To Generate the Intake Note:

  • Scroll down to the bottom of the Intake tab.
  • Click the Generate Intake Note button

Users can also collapse all panels using the Toggle Panel Controls.

Additional Billing Procedures

If a preventative medicine service or problem oriented visit is done on the same day as the PPD placement. Bill for the E&M service 99212 and higher using Modifier 25.

Return visit to have the nurse read the test

When the patient returns to have the nurse read the test (usually happens within 48 - 72 hours after test is given), to see if it is positive or negative, then bill 99211.

Remember that for Medicare, Nurse Visits must meet the criteria of incident, to bill. So a physician or NPP must be in the office to bill Medicare for this service.
