In this lesson we will explore Five simple points that will assist in minimizing chart errors. These Five points should be performed prior to viewing and documenting in patient charts. These Five points are known as the Five Point Check.

The Five Point Check consist of the following:

  1. Correct Patient - Verify that you have the correct patient. (Name, and Date of Birth)
  2. Correct Location - Verify that you have the correct Encounter Location.
  3. Correct Provider - Verify that you have the correct Provider.
  4. Correct Encounter - Verify that you are about to use the correct Encounter.
  5. Correct Encounter Provider - Verify that the correct Rendering Provider is attached to the encounter.
