This Quick Refence Guide will explain three ways to add the Reason for Visit within a patient's chart in EHR.
This Quick Reference Guide assumes the user has basic knowledge of EHR.
Adding the Reason for Visit
The Reason for Visit can be accessed from the Intake or SOAP templates.
Option # 1
One way to add the Reason for Visit is too:
1. Select from the list of pre-populated "Reasons".
You can add a maximum of up to six "Reasons" for visits.
Adding Reason for Visit using the "Other" field.
Option #2
The Reason for Visit can also be added by:
1. Free typing the desired reason in the "Other:" field.
2. Click Add

Option #3
The Reason for Visit can also be added by:
- Clicking Additional/Manage.
2. Select a reason from the pre-populated picklist, or Free type the reason in one
of the six free text fields.
3. Click Save & Close
