How to Create a BMI Plan

In this Quick Reference Guide, we will learn how to create a BMI Plan using the Health Promotion Plan template.

Accessing the Health Promotion Plan Template

The Health Promotion Plan template can be accessed from the following locations:

Vital Signs Panel (Intake MG and SOAP MG templates)
A/P Details template (SOAP MG Assessment/Plan Panel and the Navigation MG bar)

For the purpose of this illustration, we will access the Health Promotion Plan template from the Vital Signs Panel.

To access the BMI Plan template; click the Health Promotion Plan active text link.

To create a BMI Plan to address the patient's weight:

Select the Diagnosis that fits within the patient's BMI score.
Choose Status of Diagnosis.
Select a Diet plan.
Enter the Details of the Diet plan.
Select a Physical activity plan.
Document the Details for the Physical activity plan.
If referring the patient for Weight Management; enter the details in the Referrals fields.
Click Add
Click OK to close window.

The Referrals section is not a referral request. It is a plan order to refer the patient. A Referral Request will need to be completed.
