Medication Reconciliation - Quick Reference Guide

In this Quick Reference Guide we will explain the proper procedures for performing a Medication review and Medication Reconciliation. This Quick Reference Guide assumes that the user has some basic knowledge as to how to add and edit medications in the Medications Module as well as how to access the Medication Review window.

Reviewing Medications from Medications Module

To review medications using the Medications Module, click the Add/Update button from the Medications panel located on the Intake template.

After reviewing, adding, and editing the patient's medications list, click the Medication Reconciled check box then, close the Medication Module window.

Note: the Medications reconciled check box on the Medications panel is checked automatically when the Medications Reconciled check box is selected in the Medications Module.

Medication Reconciliation

Medication Reconciliation should be performed within 30 days of a patient being discharged from the Hospital, SNF facility, or Long term health facility. After reviewing a patient's medications in the Medications Module, click the Reconcile button.

Note: the Medication Reconciliation must be done prior to finalizing the visit.

Perform the following steps to complete a Medication Reconciliation in the Medication Review window:

  1. Select the discharge source by clicking the Source field from the Reconciliation Type panel. (The options include the following: Hospital, SNF, and Long Term)
  2. Click the Review All - Taken As Directed button.
  3. Click the Transitioning into care, and Summary of care received Patient status check boxes.
  4. Click OK
