The Contagion Risk link has been added to all core templates (i.e. Intake, SOAP, etc.). The link includes a badge count to inform users when the patient is at risk of having been exposed to contagions. Clicking this link opens to the Contagion Exposure template where you can document any exposures a patient has had to contagions and any areas they may have traveled with high risk of exposure.
Adding Contagions to The Contagion Exposure Template
To document a patient's exposure to contagions:
- Select the type of exposure from the Exposure grid.
- If details are known about the exposure, free text the details in the Exposure detail field.
- Indicate the risk level, by clicking the Risk field. The following options are available: high risk, low risk, some risk, and no risk.
- Document Where the exposure took place by clicking the State or Country radio buttons.
- If known enter the Exposure date.
- If the Exposure date is not known enter the Travel arrival and Travel departure dates.
- Enter the Duration of exposure if known.
- Click Add
If the patient is unwilling to answer any questions related to their exposure, click the Patient declines to answer check box.
CDC Web Link
The link to access the CDC website is available on the Contagion Exposure template. If resources are available for any exposure, the Web button becomes visible. When the Web button is selected, additional information on the exposure including the estimated Incubation period becomes available.
Updating Contagion Exposure History
If a Contagion Exposure needs to be updated:
- Select the Exposure from the Contagion Exposure History grid.
- Update the appropriate fields.
- Click Update to save changes.
Deactivating Exposures
To Deactivate an exposure:
- Select the Exposure from the Contagion Exposure History grid.
- Click Deactivate check box
- Click Update
Removing Exposures
To remove Exposures from the Contagion Exposure History grid:
- Select Exposure
- Click Remove
Travel History
The patient's Travel History can also be recorded by selecting one of the following three options:
- Out of state - Allows the recording of Where, Return date, and Duration of trip.
- Out of Country - Allows for the recording of more than one Country, Return dates for each location, and Duration of trips.
- Travel exposure - Allows the recording of any Exposures the patient may have had while traveling.
