This Quick Reference Guide outlines the proper steps for requesting Health Plan Eligibility Access.
Anyone who accesses the NextGen Practice Management Application is required to submit this request.
Accessing the Request Form
To access the Health Plan Eligibility Access Request form:
- Click the Documents tab on the Internal Website home page.
- Select New or Reset HP Password from the menu.
On the Health Plan Portal page; click the New HP Password Request option.
Select the Reset HP Password Request option to request password reset.
Health Plan Eligibility New Access Request
On the Health Plan Eligibility New Access Request form:
- Complete all required fields.
- Click the Create Request button.
Access to Health Plans not listed on this form will need to be set up individually for your staff member(s).
